Skilled and Long-Term Nursing Facilities

Our board certified Internal Medicine Physicians and Nurse Practitioners provide overall care to patients in nursing facilities, either long-term or for skilled rehab. Our Physicians are former hospitalists that are comfortable managing acutely ill patients, reducing need for unnecessary hospitalizations. Our providers are experienced with quality improvement, familiar with advanced medications, and comfort working with multidisciplinary teams to ensure the best possible quality of life and outcomes for our patients. We offer regular on-site visits and are available 24/7 for any acute needs for our patients.

Assisted Living Facilities

Our team of board certified Internal Medicine Physicians and Nurse Practitioners offer on-site care to residents within Assisted Living, Independent Living, and Memory Care Facilities. We schedule visits at regular intervals to ensure appropriate oversight and management of all problems and ensure continuity of care with any other providers a patient may be seeing. We work alongside home health, hospice, and the facility staff to ensure the best quality of care is provided. We are available 24/7 for any acute needs and offer the use of our patient portal for family members and/or loved ones to stay informed with the medical team and facility staff.

Community (Home Bound) Patients

We offer either on-site or telemedicine visits to patients in the community that are homebound that do not live within a facility setting. These visits can alleviate the need for difficult transport scheduling. Utilizing local vendors, we can also obtain lab specimens and radiology at home as well. Our team of providers ensures continuity of care with other providers that may be involved in care, including home health or hospice. Patients have access for acute needs 24/7 through our communication platform and portal.
