Transitional Care Physicians of America was founded in 2014 by Dr. Amar Mohan with the mission of transforming transitional care. Today, we are a paving the way for a new model of care delivery targeted at improving care for a population of patients historically overlooked by our healthcare system. We partner with leading hospitals, rehabilitation facilities, and assisted living communities to improve healthcare transitions. Our unique, structured care model complements primary care, maximizes care coordination, and enables early identification and response to healthcare risks. Through a comprehensive, holistic focus on patients’ needs, TCPA improves clinical outcomes, reduces healthcare costs and transforms the patient experience. Amar Mohan, MD
Chief Executive Officer
Dr. Mohan is the visionary behind TCPA's unique care model. Dr. Amar Mohan grew up in Georgia and is a board certified in Internal Medicine. Dr. Mohan has extensive experience in Palliative Care with a special interest in geriatric medicine, dementia care and treatment, psychiatric medicine, transitional care, and holistic medicine. Dr. Mohan enjoys hiking, traveling, and spending time with his wife, kids, and dog Taj.
Arun Mohan, MD, MBA, SFHM
Board Member
Board Certified in Internal Medicine, Dr. Mohan has experience building and scaling healthcare delivery organizations. As a member of the Board of Directors, Dr. Mohan helps lead TCPA's strategy and growth. Dr. Mohan received his Medical degree from Emory University and completed his Internal Medicine training at Harvard University. Dr. Mohan enjoys spending time with his wife and family and sits on the board of several philanthropic organizations.
Priti Jindal, MD, CME
Chief Medical Officer
Dr. Jindal is a board-certified internist and has over 20 years experience as a practicing physician and physician leader. Dr. Jindal was previously Chief Medical Officer at Five Star Senior Living, one of the nation’s largest operators of senior living facilities. Earlier in her career, Dr. Jindal worked as an acute care hospitalist and served in multiple leadership roles in the hospital setting.
Uzoma Anaba, MD
Dr. Anaba is Board Certified in Internal Medicine. Dr. Anaba is well versed in long term care, palliative and hospice, and hospital medicine.
Faizan Shaikh, MD
Dr. Shaikh is a Board certified in internal medicine and work as a hospitalist with unique training in geriatric medicine. He is well versed in long term, palliative and hospice care. He has been involved in various scholarly activities in hospital medicine as well as completed courses for patients requiring critical care. He enjoys playing/watching football, spending time with my family, and traveling.
Vamsi Kanagala, MD
Dr. Kanagala completed residency at Montefiore Medical Center and is a board-certified physician in Internal Medicine. He has worked as a hospitalist in New York and has experience treating an array of illnesses. Dr. Kanagala is proficient in working with a geriatric population and their families, as well as, caring for individuals requiring palliative and hospice care. His goal is to provide the upmost care for his patients and to help them regain an overall state of well-being. A native of New Jersey, Dr Kanagala recently moved to Atlanta with his wife. In his free time, he enjoys spending time with his family, traveling, hiking, attending the theatre, basketball, and swimming.
Yolanda Scott, MD
Dr. Yolanda Scott is a Physiatrist that is Board Certified by the American Board of Physical Medicine and Rehabilitation. With a background in Physical Medicine and Rehabilitation and Interventional Spine, she focuses on non-surgical musculoskeletal injuries, acute and chronic pain, degenerative changes as well as diagnostic testing for neuropathic conditions using electromyography testing. She also enjoys ultrasound-guided peripheral joint injections as well as trigger point injections for muscle tender points and acute muscle spasms. Dr. Scott’s training focuses on creating a collaborative treatment plan to improve a patient’s physical limitations. Her goals are to improve conditioning and provide an opportunity to return to an optimal lifestyle. During her free time she enjoys spending time with her family and friends, exploring the city of Atlanta, traveling, running in 5K events, and cooking.
Madhu Kanagala, MD
Dr. Kanagala is a dual board certified physician in both Internal and Geriatric Medicine. His vast experience in working with these populations has spanned over 30 years and includes acute, post transitional, and long term care. In addition, his experience as a hospitalist has made him proficient in providing the utmost care while balancing patient autonomy for individuals receiving palliative and hospice care.
In his free time, Dr. Kanagala enjoys spending time with his family, traveling, and watching movies.
Urviben Patel, MD
Dr. Patel is a dual certified physician in Internal Medicine and Palliative Care. Dr. Patel comes with a wealth of experience from both the hospital and outpatient world.
Kobena Waters, MD
Dr. Waters is a recent graduate of Emory University School of Medicine. He is originally from Maryland and is an alumnus of the University of Maryland. In his role as Clinical Liaison he works to facilitate and improve communication between members of the healthcare team, patients, and their families with the goal of improving patient's experience, outcomes, and satisfaction. He is interested in the development and deployment of technological solutions to enhance healthcare delivery, with plans to practice in Physical Medicine and Rehabilitation.
Dr. W. Dennis Taylor, MD
Dr. Dennis Taylor is the founder of Southeast Geriatrics. Dr. Taylor is an experienced gerontologist, having completed medical school at The Medical College of Georgia and his residency at Memorial Health – University Medical Center in Savannah, GA.
Al Warren, MD
Dr. Warren is originally from Cordele, Georgia. He served in the United States Navy. He attended Georgia Southwestern State University for undergraduate studies where he served as a Pope Fellow for the Rosalyn Carter Institute for Caregiving. He received his Doctor of Medicine degree from Mercer University School of Medicine in Macon, Georgia. He also completed his internship and completed his residency in Family Medicine at the Medical Center of Central Georgia/Mercer University Program. Dr Warren is board certified in Family Medicine and has a passion for Geriatric Care. Dr. Warren has experience in Emergency Medicine, Urgent Care and Primary Care. Dr. Warren is married with four children and three grandchildren. He enjoys outdoor activities and woodworking.

Dr. Hooda is a dual certified physician in Internal Medicine and Palliative Care. Dr. Hooda comes with a wealth of experience from both the hospital and outpatient world.

Roberta “Bobbi” Walters, ARNP
Roberta "Bobbi" Walters is a nationally certified Family Nurse Practitioner. She graduated from The University of Florida with a Bachelors of Music in Performance with Honors, from Emory University with a Bachelors of Science in Nursing, Summa Cum Laude, and Columbus State University with a Master of Science in Nursing in 2017. After spending time during her clinical rotation with TCPA, she joined our team shortly after graduation. Bobbi is a member of the American Association of Nurse Practitioners, Sigma Theta Tau Nursing Honor Society, and Golden Key International Honor Society.
Ericka Swilling-Nobles, FNP-C
Ericka Swilling-Nobles is a board certified Family Nurse Practitioner. She graduated from Mercer University with a B.A in Sociology. She then went on to obtain her Associate of Science in Nursing at Macon State College; Bachelors of Science in Nursing from Georgia College and State University, and Masters of Science in Nursing from Kennesaw State University. She is member of the American Association of Nurse Practitioners. * Ericka is Georgia native and brings a wealth of experience in long term care, palliative care, and hospital medicine.
Marsha “Davette” Watson, FNP-C
Marsha "Davette" Watson is a board certified adult nurse practitioner. She graduated from Jacksonville State University with a Bachelor of Science in Nursing, and Chamberlain College with a Masters of Science in Nursing as Adult Nurse Practitioner in 2017. She joined TCPA in 2017 and currently serves our patients in the Rome, GA area. She currently holds certifications in Case Management, Chemotherapy, and is a member of the American Association of Nurse Practitioners.
Alexcia James, DNP, AGNP, ARNP
Alexcia James is a board certified family medicine and geriatrics nurse practitioner. She graduated from The University of Iowa with a Bachelors of Science in Nursing (BSN) with Honors and The University of Iowa with a Doctorate of Nursing Practice as Adult Geriatric Nurse Practitioner in 2013. Alexcia is a member of the American Nursing Association, Preventative Cardiovascular Nurses Association, and United Advanced Practice Registered Nurses. She joined TCPA in 2016. In her free time, she enjoys spending time with her family and her furbaby Buddy.
Hila Oren, MSN, APRN, AGNP-C
Hila Oren is a board certified Adult and Geriatric Nurse Practitioner. She graduated from Mercer University’s Georgia Baptist College of Nursing with Honors and University of Alabama in Birmingham with a Master of Science in Nursing with a Geriatric specialty in 2015. Hila is a member of Sigma Theta Tau Nursing Honor Society, Gerontological Advanced Practice Nurses Association (GAPNA), and American Association of Nurse Practitioners. In her free time, she enjoys spending her weekends enjoying festivals and events with her family including her husband, daughter, and their four dogs.

Amy Sargent is a board certified family nurse practitioner. She graduated from Georgia College and State University with a Bachelor of Science in Nursing in 2003, and South University with a Masters of Science in Nursing as a Family Nurse Practitioner in 2015. She joined TCPA in 2018 and currently serves our patients in the Monroe, GA area. She is a member of the American Association of Nurse Practitioners.

She completed her senior practicum at Salude and joined TCPA after completing her masters program. Emily Adams, FNP-C
Emily is a board certified Family Nurse Practitioner. She graduated from Kennesaw State University with a Bachelor of Science in Nursing and from Simmons University with a Master of Science in Nursing. She has a passion for improving the care of patients with advanced heart failure and her research has been published in Heart & Lung: The Journal of Acute and Critical Care. Emily joined TCPA in 2019. She is a member of the American Association of Nurse Practitioners and the United Advanced Practice Registered Nurses of Georgia.
Andrea Morgan, APRN, FNP-C
Andrea is a board certified Family Nurse Practitioner. She graduated Magna Cum Laude with a Bachelor of Science in Nursing from Armstrong Atlantic State University. She continued her education with Walden University, graduating with honors with a Master of Science in Nursing. She joined TCPA in 2020. She is a native of Toombs county and will be serving patients in her hometown and nearby Treutlen county. She brings over 15 years of bedside experience and is passionate about preventative care and improving patients' overall quality of life. In her free time, she enjoys hanging out with fiancé, Ryan, and their fur babies, Bonnie and Bubba.
Deven Szadek, MSN, NP-C
Deven Szadek is a board certified family nurse practitioner. She graduated from Georgia Regents University with her Bachelor of Science in Nursing and University of North Georgia with a Master of Science in Nursing in 2019. She joined TCPA in 2020. She is a member of the American Association of Nurse Practitioners. Away from work, she enjoys practicing yoga and spending time with her family and friends.
Bolanle Babajide-Gbenga, PMHNP
Bolanle Babajide-Gbenga (Bolan) received her Master of Nursing Degree and Bachelor Degree from The Walden University and Valdosta State University in Valdosta Georgia respectively. Bolan is a board certified Psychiatric Mental Health Nurse Practitioner. In her free time, she enjoys singing, dancing and spending time with her family.
Laura Mullis, APRN-BC
Laura Mullis graduated with her Family Nurse practitioner degree in 2004 from Georgia College and State University and is also certified in Advanced Practice of Hospice and Palliative Nursing. She worked for 10 years at Navicient Hospital in Macon as a Palliative care Nurse practitioner and was adjunct faculty for Mercer University School of Medicine for their Geriatric and Palliative fellowships. She also served on the board for Central Georgia Hospice as they opened their Inpatient Hospice unit. She has been with SE Geriatrics for over 7 years and works in various facilities across Central Georgia.
Shelley Pool, FNP-BC
Shelley Pool is a board certified family medicine nurse practitioner. She graduated from Georgia College with a Bachelors of Science in Nursing, and then from Georgia College and State University with a Masters of Science in Nursing as a Family Nurse Practitioner. She has worked in long term care for the majority of her Nurse Practitioner career. Shelley is a member of the Georgia Medical Directors Association. In her free time, she enjoys spending time on the lake with her family and friends and vacationing at the beach.
Brandi Roberson, FNP-C
Brandi Roberson is a Board Certified, Family Nurse Practitioner. She currently resides in Milledgeville, Georgia where she was also born and raised. She has 13 years of diverse clinical experience in the health care field that includes long-term care, geriatric, and hospice care. She completed her education at Darton University College receiving an Associate’s Degree of Science in Nursing and at Chamberlain University where she received her Bachelor’s and Master’s degree of Science in Nursing. She was inducted into Lambda Chi Nu at Darton College Honor Society in 2011. She began her career in 2003, working as a nursing assistant in a long-term care facility. While working there, she developed her passion of caring for the geriatric population. She was inspired to obtain her practical nursing degree in 2007 and Registered Nursing Degree in 2011. Determined to improve her professional responsibility and desire to provide primary care she received her Nurse Practitioner Certification in the years to follow. She is driven to provide patient-centered care that provides an environment that promotes growth and quality.
Amanda K. Smith, NP
Amanda graduated from Georgia College and State University in 2009 with a Master of Science in Nursing, Family Nursing Practitioner degree, and is Board Certified with the American Association of Nurse Practitioners (AANP). Amanda began her career as a Nurse Practitioner working 6 years in an Infectious Disease and Wound Care practice in Acute Care, Long Term Acute Care, and office settings. She has 5 years of experience in long-term and geriatric care. She is a member of American Association of Nurse Practitioners and the Georgia Medical Directors Association. Amanda has been a team member with Southeast Geriatrics since 2019
Shannon Coffee, FNP
Shannon "Dawn" Coffee is a board certified family nurse practitioner. Dawn graduated from South Georgia College in 2000 with an Associates of Science in Nursing. She completed a Bachelor of Science in Nursing from Georgia Southern University in 2013 and a Master's of Science in Nursing from South University in 2016. She has been working in geriatrics and long term care since 2009. She has worked as an FNP with a geriatric patient population since 2016. Dawn joined TCPA in 2021 and serves patients in Douglas and Waycross. She is a member of the American Association of Nurse Practitioners, Georgia Medical Directors Association, and Gerontological Advanced Practice Nurses Association. In her spare time, she enjoys spending time with her husband and 3 children, traveling, and going to church.
Brooke Evans, FNP
Brooke Evans began her nursing career as a LPN at Irwin County Hospital. She had several years of hospital experience as both a LPN and RN. She has a few years experience in nursing education as a Practical Nursing Instructor. During this time, She instructed students in the classroom, lab, hospitals, and long term care homes. Brooke completed her MSN degree through Walden University. She and her husband have two children. During her spare time, Brooke enjoys spending time with her family, fishing, and being outdoors. Brooke hopes to someday be able to travel more.
Danielle Bruce, FNP
Danielle is board certified as a Family Nurse Practitioner. She graduated from Brenau University with a Bachelor’s of Science in Nursing in 2013. She worked as an ICU nurse for seven years and obtained her CCRN. Danielle received her Master’s of Nursing Science from South University in 2020 and has worked in Internal Medicine and as a hospitalist nurse practitioner. In her free time, Danielle enjoys spending time with her family and working in the yard.

Kayla Smiley, FNP
Kayla Smiley is a board certified Family Nurse Practitioner. She graduated from Augusta University with a Bachelor of Science in Nursing, and South University with a Master’s of Science in Nursing in 2021. Kayla joined TCPA in 2021 and is a member of the American Association of Nurse Practitioners. She currently serves patient in the Conyers area. Outside of work, she enjoys spending time with her family and friends.

Melina is a board certified Family Nurse Practitioner. She graduated from Georgia Regents University (previously Medical College of Georgia) with a Bachelor of Science in Nursing in 2015. As a nurse, She has experience in Geriatrics, Pulmonology, and Surgery. Melina graduated with her Master of Science in Nursing from South University in 2019 and has worked as a hospitalist nurse practitioner. Brittney is a board-certified Family Nurse Practitioner. She has eight years of health care experience, with the last two years working in primary care. She received her Bachelor’s of Science in Nursing Degree from Georgia Southern University in 2013 and a Master’s degree from South University in 2019. She joined TCPA in 2021. Brittney is compassionate, caring, and has a passion for providing high quality health care. Born and raised in the beautiful Golden Isles, she resides there with her husband and two sons. She enjoys being outdoors and spending time with her family and children.
William Patton, AG-ACNP
William Patton is a board certified Adult/Gerontology Acute Care Nurse Practitioner who graduated from Walden University in 2022 w/ Masters in Nursing. He has 5 years MSICU experience and has accumulated 13 years of medical experience w/ military field medicine, Ems, and nursing. William is an Army veteran, who served a 12 month tour in Afghanistan as line medic May ’09-May ‘10. His free time is spent with his fiancée and 3 children having outdoors adventures including hiking and exploring. William has also participated in competitive strongman with multiple Georgia state titles.

Christina “Chrissy” Tyler, FNP
Christina "Chrissy" Tyler is a Family Nurse Practitioner that is board certified and is a current member of the American Association of Nurse Practitioners. She grew up in Clayton, Georgia then attended the University of Georgia. She then graduated from Brenau University in 2000 with her BSN after being voted Class Historian and Representative and started her Registered Nursing career which mainly consisted of critical care. She became a travel nurse after a few years and after working in numerous major hospitals around the United States, she decided to come back to Georgia and attend North Georgia College and State University and she obtained her Master's of Science in Nursing/FNP degree. There she graduated with Presidential Honors and was inducted into the Sigma Theta Tau's International Nursing Honor Society and published her thesis on "The Prevalence of Diabetes in Children." Her Nurse Practitioner career started in 2006 where she has worked in numerous fields over the years including primary care, emergency medicine, urgent care, hospitalist medicine, med spa, and hospice. She has certifications that include DOT examiner, Cardiac stress test examiner, Aesthetic injection specialist, Hyaluronic acid knee injections. In her spare time, she enjoys spending time with family and friends relaxing on the river and lake and traveling.
Elizabeth “Davis” Callahan, NP-C
Elizabeth “Davis” Callahan is a board certified Family Nurse Practitioner. She graduated from The College of Coastal Georgia with a Bachelor in Nursing, and then went on to pursue her Master of Science in Nursing at Valdosta State University. Davis is a member of the American Association of Nurse Practitioners (AANP). In her free time, Davis enjoys spending time with her family and friends, and going to the beach.

Britny Catlett, LPN
Chief Operating Officer
Britny is a graduate of Trident Technical College in Charleston, SC as an LPN and moved to Georgia in 2013 to start her nursing career after multiple years of management in other fields. After several years in various nursing settings including assisted living, skilled nursing, and specialist offices, she joined the TCPA team. As the current Chief of Operations, she oversees the daily operations of the office. She is currently working on her Masters program. When not working, she enjoys spending time with her husband and three sons.
Vadivan Mani, MBBS
Director of Clinical Compliance
Vadivan has been with TCPA since the beginning. He is a medical graduate from Sri Ramachandra University, India. His experience in the clinical and business aspect of health care helps assist and educate our staff. He enjoys spending time with his family and being involved in outdoor activities in his free time.
Kristina Blevins, LPN
Kristina graduated from Georgia Piedmont Technical College as a Licensed Practical Nurse in 2017 and joined TCPA shortly after graduation. She was raised here in Georgia and enjoys spending time with her husband, son, and new daughter.
Amberly Galbraith, LPN
Amberly is a transplant from the Carolina’s where she completed nursing school. She is a part of our back office support staff assisting with providers, patients, and families. She brings years of experience in home health care with background in geriatric care. Her family, any time near salt water, and getting lost in a book bring her joy.
Cody Rowell, LPN
Cody is from a small town in Alabama called Hollins. After graduating from Fortis College for nursing in Birmingham, he moved to Georgia in 2018 with his fiance. He joined TCPA in 2020 after years of experience including long term care, wound care, and home health. When not working, he enjoys traveling and spending time with his family and dogs
Elizabeth Norris, MA
Elizabeth is a Medical Assistant born and raised in Georgia with a medical background in geriatric care and dermatology. She enjoys spending time with her three children and her lab puppy. Elizabeth joined TCPA in 2018.
Katelyn Carter, MA
Katelyn is our certified medical assistant. She recently moved to Georgia from Indiana and joined TCPA in 2018.
Sandra Ennis
Sandra has over 10 years experience with insurance credentialing and contracting. She was born and raised in Southern California to which she still calls home. She enjoys cooking and baking in her spare time for her fiancé and three children, watching movies, and listening to good music with friends.
Jessica Corneille, MA
Jessie has over a decade of healthcare experience in internal medicine, geriatrics, urgent care, infectious disease, and gen. surgery. When not working, she enjoys spending time with kids and husband traveling.
Ashley Lunsford
Ashley was raised in Georgia and attended Gainesville State College. She has a varied background in management and over 6 years expertise as an administrative assistant. When she is not working, she enjoys her time with her husband and two boys in the outdoors hunting, camping, and working toward her advanced open water scuba certification.
Haleigh Adams
Haleigh is a Certified Medical Assistant. She graduated from New Horizons Medical Institute in 2016 with a medical background in obstetrics and gynecology. She joined TCPA team in 2020 and assists in the back office. In her off time she enjoys spending time with her husband and twin boys.
Andrine Toussaint, LPN
Andrine is an experienced LPN who worked primarily in long-term care and geriatrics. She graduated from Lorenzo Walker Technical College (Naples, FL) in 2004 and relocated to Georgia. She enjoys spending time with her family and relaxing in nature. Writing poetry is her passion and her favorite activity outside of work.
Laura Buckles, MA
Laura is a certified medical assistant. She graduated from Gwinnett College in 2013 and joined TCPA in 2019. She is a Georgia native and mother of 3 , she enjoys spending time with her kids and snuggles with her Dog Bella.
Samantha Livsey
Samantha was born and raised in Georgia. I have 5 years experience of working in the medical administration field. In my spare time I enjoy crafting, and spending with my fiancé and friends. I also enjoy spoiling my newly adopted rescue dog.
Kalyani Athimuthu, MS
Kalyani handles all of our insurance needs. She has been with TCPA since 2017. She graduated from Southern Tech with a Computer Science degree. While she has an IT background, she chose to enter the healthcare industry through this opportunity. She lives in Alpharetta with her husband and two sons whom she enjoys spending time with.

Brindha Balaguru has her Masters in Social Work from the University of Georgia. She has over 7 years of experience as a clinician at Georgia Regional Hospital-Atlanta. Brindha enjoys reading, watching sports and spending time with her family.
Devan Patterson, MA
Devan Patterson attended New Horizons to complete her MA degree and is currently a student at Brenau University. She is a mother to a beautiful 2 year old and enjoys spending time with family and friends when she is not working. Devan has a major love for the beach, lake and anything in between.
Brooke King, MA
Brooke King was born and raised in Georgia. She is a graduate of Lanier Technical College and received her diploma in Medical Assisting. She has been working in the Medical field for 6 years now. Brooke started off in the ER of Northeast Georgia Medical Center performing EKG'S, then moved to a cardiology office for 4 years specializing in heart failure and most recently to an internal medicine office for the last two years. Brooke is married and has two children. In her spare time she likes to enjoy time out on the lake with her family and also with her black lab.
Kendall Seaton, MA
Kendall was born and raised in Georgia. She graduated from New Horizons Medical Institute as a Certified Medical Assistant in 2017. She has worked in the Pulmonary field for about 3 years. When Kendall is not at work she enjoys cooking, baking, and crafting! She enjoys spending time with her boyfriend and their dog.
Leslie Hood, CMA
Leslie is a certified medical assistant born and raised in Georgia. Background includes working with newborn babies, internal medicine and vascular surgery. I enjoy spending time with my husband and children, gardening, knitting, spending time in nature, and learning holistic medicine.
Kayla Bridger, MA
Kayla graduated as a certified medical assistant at Athens Technical College in 2013. She joined TCPA in 2021 with a background in cardiology and pulmonary. She has a passion for helping others and becoming friends with everyone she meets. Trillan Carter was born and raised in Georgia. She is a Registered Medical Assistant and graduated from Sanford Brown College in 2011 with a medical background in ENT, Pediatrics and Geriatrics. When she is not working she enjoys being a travel baseball mom and spending time with her husband and son traveling.

Manning graduated from the Medical Institute of Northeast Georgia with a Medical Assistant Certification in 2020. She has worked for Hillcrest Foods inc. (Waffle House Franchise) for the past 7 years. As an Area People Director, she was over recruitment in the main office but also trained at her position in the restaurants. Manning has always wanted to be in the medical field and decided to pursue her dream. In her free time she loves spending time with her husband and their daughter. They love the outdoors including hunting, fishing, and spending time with close friends and family.
