PHYSICIAN - Atlanta, GA
We are looking for full-time physician to join our growing team on the east side of Atlanta. The physician will lead a team of Nurse Practitioners who are embedded in skilled nursing facilities through the region. The physician will be a key part of our structured, collaborative care model that is transforming the delivery of healthcare in post-acute care settings and that improves clinical outcomes and patient experience.
Our unique model provides physicians with unprecedented flexibility and market-leading income. You will work at your own pace and have the support of a team of advanced care providers and administrative staff. Physicians will be eligible for ownership in the practice after 2 years of successful service.
TCPA is committed to improving quality outcomes, reducing avoidable readmissions and enhance the patient experience. With a high-touch approach, TCPA helps ensure proper progress on plan of care and identify any early warning signs for readmissions during the critical, immediate post-discharge period.
Job Responsibilities
Responsible for admissions and for executing the post discharge plan of care
Responsible for ensuring safe discharge to home from the skilled nursing facilities
Provide a true continuum of care to our patients alongside our facility partners
Oversees nurse practitioners providing transitional care services in skilled nursing facilities

Our post-acute program will provide extensive administrative and logistical support to the providers as well as portable technology, mileage reimbursement, flexible scheduling, and a 4 day work week regarding patient face to face encounters
Minimum Qualifications
MD or DO
Minimum of 3+ years of experience working with geriatric population - preferred
Board certified or board eligible in internal medicine, family practice or geriatric medicine
Ability to lead and work in a team
Excellent communication (written and verbal), analytical and problem-solving skills
Entrepreneurial mindset with ambition to transform post-acute care delivery
